Main Purpose of the Job  The purpose of this role is to provide support for service users to access or re-engage with appropriate services and groups in both the statutory and voluntary sectors to help them manage their own health and wellbeing.  The Link Worker will meet with referred service users, carry out holistic assessments and explore options with people to help them meet their personal goals.  In addition, the post-holder will proactively develop and support groups that promote peer support and self-care to become established in the community.  

WORKING RELATIONSHIPS The post holder is responsible to the PEP Lead Practitioner for overall individual performance and day to day duties The post holder will work closely with the PEP team and the wider Families and Communities team at BLC.
