Care & Repair Leeds is the Home Improvement Agency for the city, highly regarded for providing a range of services for older people and disabled people to help them to remain living independently in their own homes.
As an independent Registered Society with charitable status covering the whole of the Leeds Metropolitan District area, we have been providing services since 1987.
We are expanding our Support Worker team and seek to appoint a Support Worker to work 3 days a week, with the possibility of some additional hours in the short term.
You will work closely with our partners, Age UK Leeds and Groundwork, delivering the Home Plus Leeds service.
This involves visiting people in their own homes to provide advice, guidance and support on home based interventions to improve their health and wellbeing, enabling them to remain living independently in their own home for as long as possible, including assessing for and delivering items of equipment to prevent falls, such as raised toilet seats and bath boards.
You will also be involved in an exciting new project providing an information and advice hub for patients and their families/carers in St James Hospital. We take a person-centred approach, focusing on the needs of the individual, gathered by having a conversation with them (and their families/carers), to find out what will really make a difference to them in their home.
You must have proven experience of working within a casework and/or support worker role within a community setting; knowledge and appreciation of the housing issues experienced by older people, disabled people and people with health conditions which impact on their ability to live safely in their own home.
You will have experience of supporting people with a wide range of needs, communicating sensitively and effectively to identify their needs and arranging for appropriate interventions to be put in place, and/or signposting to other agencies, ensuring a more sustainable outcome for the client.
You will be IT proficient, including the use of Microsoft Office programmes, and be physically able to lift and carry items of falls equipment such as raised toilet seats and bath boards.
