Hospital to Home Support Worker Age UK Leeds is a leading local voluntary sector organisation providing high quality services to improve the health and wellbeing of older people in Leeds.
Scale 4 £19,554 - £21,166 pro rota
25-28 Hours per week, on a 7 day rota
As a support worker you will be responsible for; supporting families/carers to provide care pending implementation of a package of care (POC) enabling an earlier discharge and preventing readmission.
Providing low level needs care within their home.
Providing support to individual clients to facilitate a timelier discharge from the wards. Transporting clients from hospital to their home and assiting them to settle back in. Transporting clients into the hospital to attend outpatient appointments.
The successful candidate will have the ability to Show good understanding of the needs of older people.
Driving licence and own vehicle is essential for this post.
Experience of Lone working in the community and within a hospital environment would be of a distinct advantage.
You will be based at St James University Hospital, Leeds/ In the Community
