You will coordinate additional support by working closely with the discharge teams within the hospital for those patients who may require some short-term support within the community for their mental health, substance use or frailty, aligning them to the correct workers within the MAST team. The job will also include outreach work to clients, working with the community-based team to provide person-centred support within their own environment.
