A new 'social prescribing ' service commissioned by NHS Leeds South and East Clinical Commissioning Group has gone live.
To help people in South and East Leeds access a wide range community groups and activity to improve their health and wellbeing. All patients registered with a GP practice in Leeds South and East will be eligible to access the free service, called Connect for Health.

The service will offer an opportunity to tackle the wider issues that can impact on health and wellbeing in a way that cannot always be solved through traditional medical routes alone. Patients can be referred by their GP or other health professional, or can self-refer via the website www.connectforhealthleeds.org.uk or by calling 0113 3876380.
NHS Leeds South and East Clinical Commissioning Group will be working with the service provider (a consortium of organisations led by Leeds Mind) to promote the service among GPs, other health and social care professionals and the public.
An official launch will be scheduled on January 25th, 1-3pm at the Connect for Health Offices, Hillside, Beeston. It will provide a chance for stakeholders to meet the Connect for Health team and hear from patients who have accessed the service within its first few weeks.
