Join the Twitter discussion from 2pm-3pm on Tuesday 22 March to see how the NHS can help people to manage their health better.Members of the public are being invited to join a Twitter discussion from 2pm-3pm on Tuesday 22 March to see how the NHS can help people, and in particular those with long term conditions, manage their health better.
The conversation is being hosted by NHS Leeds West Clinical Commissioning Group (CCG). The CCG is looking at ways to improve support available for people, especially those with long term conditions, to manage their health better.
The session will be led by a local GP and a commissioning manager from the CCG. They will be on hand to respond to any thoughts, concerns and questions about their plans to introduce health coaches.
Health coaching aims to give people the knowledge and skills to take control of their long term condition so that they can reach self-identified goals. This approach sees patients being supported by health coaches. Health coaches offer support through regular follow-ups so that people have the tools needed to look after their own health.
