Local Care Partnership - Winter Planning

The Local Care Partnerships (LCPs) in Leeds are delivering a series of local workshops to help coordinate planning and ensure all communities are supported this winter.

Local Care Partnership is the term adopted in Leeds to describe the model of joined-up team working to improve health and care delivery to local people, a requirement set out in the NHS Plan.

The LCP Development Team are delivering a series of Winter Planning Workshops, across each of the 15 areas of Leeds, to communicate Citywide plans and to allow Third Sector organisations working with diverse communities to coordinate their support with system partners.

Dates are being confirmed, but if you would like to join one, or more, of these workshops please complete the brief expression of interest form below. https://forms.gle/JzvMgXh7sKDW5YQ59
