Forum Central and Leeds Community Foundation are working in partnership with funding from NHS Leeds Clinical Commissioning Group (CCG) to ensure the Third Sector is better connected, influential and resourced and an active, integral part of local health and care systems transformation and systems-wide culture-shift.

Now in its second year, the Small Grants programme is continuing to focus on developing, strengthening and sustaining diverse Third Sector community activity that meets Leeds System health and care priorities and aligns with the Leeds Health and Care Plan.
