Leeds Community Mental Health Transformation

Added: 16/06/2023

Mindwell Leeds

The vision for community mental health care in Leeds is to create a radical new approach of joined up primary and community mental health services and support that respond to local people’s needs and remove barriers to access.

  • People will be able to access care, treatment and support as early as possible.
  • Live as well as possible in their communities

This vision is being delivered by the NHS in Leeds, Leeds City Council and voluntary organisations working in partnership. Called the Leeds Community Mental Health Transformation, the programme has adults and older people with ongoing and complex mental health needs(severe mental illness / SMI) in its scope.

Working across all sectors, we have developed a model for the future of community mental health care in the city:

Delivering the vision

Working across all sectors, we have developed a model for the future of community mental health care in the city:

  • We are delivering transformation by building on the good stuff we already have in Leeds Primary Care Mental Health.
  • There will be simple referrals.
  • People accessing the services will have an initial helpful conversation to discuss their needs and how they can best be met.
  • We are using a trauma–informed approach.
  • Existing offers will be integrated across primary care mental health, community secondary care and broader social support and multi-agency care.
  • All of this will be delivered by a multidisciplinary team.

Integrated hubs

At the heart of our new model are integrated community mental health hubs. These are the multidisciplinary teams that will deliver services in our communities so that people have quick and easy access to what they need.

We are piloting three of these hubs during 2023 and by testing and learning, will extend the facilities across the city by the end of 2024. The local care partnership (LCP) early implementer sites are:

  • West Leeds
  • HATCH (which includes Burmantofts, Richmond Hill and Harehills and Chapeltown)
  • LSMP and the Light.

And the aim is to start testing and learning this autumn.
