What is the Partnerships Leading Care Fellowship?

The Partnerships Leading Care (PLC) Fellowship is a 12‑month, system‑wide development programme created for people who want to grow their system thinking, strengthen their leadership practice, and make a meaningful difference across the Leeds Health and Care system.

As a PLC Fellow, you’ll step beyond organisational boundaries and become part of a supportive, cross-system community committed to tackling real challenges in neighbourhood health. Through a project‑based, hands‑on approach, you’ll learn how to navigate complexity, influence across diverse partners, and create solutions that work for the whole system — not just one part of it.

This is your opportunity to:

  • Build confidence as a system and partnership leader
  • Apply system thinking tools to understand challenges from multiple perspectives
  • Lead a high-impact project improving health and wellbeing at neighbourhood level
  • Collaborate with colleagues from across the city’s health, care, voluntary, and community sectors
  • Develop the behaviours and mindset needed to lead change across a whole system

Who is it for?

The Fellowship is open to staff in paid employment across the health and social care, regardless of seniority.

What you need to know about the fellowship:

To take part in the programme you must have a project you want to develop and complete within the 12-month programme. Your project must align to one of the three themes below:

  1. Prevention of ill-health, Health Independence & Community Resilience - Supporting people to live well and independently in their communities, while reducing reliance on statutory services. Example project topics may include those listed below, though we also welcome your own ideas.
  • Independent living: personalised care, assistive technology, and community-based rehabilitation.
  • Community and carer networks: strengthening local support, partnerships with voluntary or faith groups, and peer support for carers and residents,
  • Wellbeing and prevention: early intervention for frailty, long-term conditions or social isolation, and neighbourhood activities that improve wellbeing and reduce service demand.
  1. Health and Wellbeing Innovation, Digital & Policy Translation – Using technology, data and improvement approaches to enhance care, translate policy into practice, and support innovation.  Example project topics may include those listed below, though we also welcome your own ideas.
  • Developing digital tools for care planning, remote monitoring or assistive technology.
  • Using data to identify neighbourhood needs, gaps and inefficiencies.
  • Turning policy into practical guides, tools or workflows for frontline teams.
  • Co-designing new care models with residents, carers and multidisciplinary teams.
  • Applying rapid improvement methods and sharing learning to scale what works.
  1. Health Equity, Inclusion & Wellbeing -  Promoting equity, inclusion and wellbeing for residents and the health and care workforce across neighbourhoods. Example project topics may include those listed below, though we also welcome your own ideas.
  • Improving recruitment, retention and progression for underrepresented groups in social care.
  • Culturally informed care initiatives to reduce health and social inequalities in local communities.
  • Workforce wellbeing projects, such as flexible working or psychological safety initiatives.
  • Identifying gaps in access to services and designing targeted support for vulnerable populations.

What’s involved?

  • 12-month programme starting April 2026 with a face-to-face development day (22 April 2026)
  • One day per month to work on your project with monthly mentor support
  • Monthly virtual development sessions and a celebration event in March 2027
